The National Transportation Safety Board (NTSB) has concluded that a single loose wire was a critical factor in the sequence of events that led to the container ship Dali striking and collapsing the Francis Scott Key Bridge in Baltimore. Investigators found that a wire-label banding prevented the wire from being fully inserted into a terminal block, resulting in an inadequate connection. The investigation also highlighted that the bridge's vulnerability to collapse from a vessel impact was a contributing factor to the severity of the disaster.
